1. Our office uses a computer-based method to store medical records. Each new patient is required to fill in a medical history online before her first visit. Click the DigiChart icon below to get started.
2. If you are unable to complete this history online, please arrive at least 45 minutes prior to your scheduled appointment time to complete it on one of our computers.
3. New patients should also fill out the New Patient Form and HIPPA Form prior to the first visit.
4. New patients must bring a current Insurance Card and Driver's License (or another form of photo ID). Please note that a new federal law requires that the address on the Driver's License or photo ID must be current. A utility or other bill can be used to verify your address if your ID is not current.
